​​​What Do We Need to Apply? 

Before the application opens you will want to gather the following items: 

​

• Your business’ complete 2023 Income Tax return as submitted to IRS

• Current City of Chicago business license

• W-9 Form signed in 2025

• Secretary of State Certificate of Good Standing (if entity other than Sole Proprietor)

• Current, valid, government issued identification document (drivers license, state ID, passport, citykey ID)

• A security assessment done by the 24th Police District

• Using the Security Assessment you will need to get two quotes for Violence Prevention and Business Protection physical interventions. (Parts/Labor/Costs etc) to upload to application.
